enhanced care management (ecm)

What is ECM?
Enhanced Care Management (ECM) is a service for eligible Medi‑Cal members who have very complex health and life situations — not just medical problems, but things like housing, mental health, substance issues, or social supports. It’s called “enhanced” because the help is more intensive: you get a lead care manager who coordinates and helps you with many parts of your life, not just doctor visits.
How it Works
You get one personal care manager (also called a Lead Care Manager) as your main point of contact. This person helps pull together all the services you need — physical health, mental health, dental, social help, housing, etc. The care manager meets you “where you are” — which might be your home, a shelter, your doctor’s office, or somewhere else. It’s to make sure the support works in your real life. Care managers can help get you connected to medical care, help you understand medications, coordinate transportation for medical appointments, link you to resources, and more.

Who is ECM For?
People who qualify are the following “Populations of Focus,” such as:
  • Experiencing homelessness
  • Frequently using emergency rooms or hospitals when fewer resources might have prevented that use
  • Those with serious mental illness, or substance use issues
  • Children or youth with serious emotional disturbance or who are at high risk for psychosis
  • Children or youth who are involved in Child Welfare
  • Adults living in the community who are at-risk for long-term care institutionalization
